Step 1: Click on "Forgot Password" (This will initial the process of re-setting your password for MyChart, which is necessary to activate your account).
Step 2: Enter your existing Duke Health View User Name in the window that asks you for your MyChart User Name.
Step 3: Enter your date of birth and your Duke Medical Record number, and then click "next." (If you do not know your Duke Medical Record number, call Duke Medicine Customer Service at 919-620-4555 or 1-800-782-6945 from 8:00am-5:00pm ET on Monday, Tuesday, Wednesday and Friday and from 8:00am-4:00pm ET on Thursday).
Step 4: Answer security questions (which should be the same as those used in your Health View account). Once this is complete, you should be able to create / reset your password for your Duke MyChart Account.

Duke Medicine has rolled out a new, state-of-the-art medical records technology which creates a single electronic record for each patient and can be accessed at any Duke location, as well as by affiliated and referring physicians. The technology creates a seamless, real-time and secure communication among all members of your Duke care team.
With the introduction of the new electronic system, the online tool Duke HealthView was transitioned to Duke MyChart. This new tool gives you access to information from your electronic medical record.

As a previous Duke HealthView user, please know that in Duke MyChart you will still be able to view your appointment information, request appointments, view medical reports, and pay your bills.
You can continue to use Duke Healthview to pay bills for services provided at Duke Raleigh Hospital, Duke Regional Hospital (formerly Durham Regional Hospital), or for any other services provided before June 22, 2013, at other Duke Medicine hospitals and clinics or any Duke Private Diagnostic Clinics.
